Psychiatric clinicians are often asked to treat patients with evidence-based methods, but the evidence to guide practice does not always reflect the reality that they confront every day.

Currently, many patients are using cannabinoids, particularly cannabidiol (CBD), to treat their own symptoms, ranging from insomnia to anxiety to depression. What doses of CBD should they be using? What route? How will CBD interact with other medications?

The mammalian endocannabinoid system includes the neurotransmitters anandamide (AEA) and 2-Arachidonoylglycerol (2-AG), their synthesizing and degrading enzymes, and their activity as partial and full agonists, respectively, at the Cannabinoid 1 and 2 (CB1, CB2) receptors. How this neurotransmitter system works as a retrograde signaling mechanism to fine tune other neurotransmitters, such as serotonin, norepinephrine, dopamine, and GABA. In clinical settings, finding the action of CBD on the endocannabinoid system will be an attempt to make rational choices about the role of exogenous cannabinoids in psychiatric practice.

CBD works by interacting with the body’s own regulatory system called the Endocannabinoid System (ECS).

The ECS plays an essential role in keeping the body functioning properly within a narrow range of operating conditions, and controls important functions like mood, memory, stress, sleep, behavior, appetite, pain, immune function and reproductive health. CBD can bind to these receptors in our bodies and activate the Endocannabinoid System, thereby regulating the same functions.

The ways CBD work in the brain are complicated and research is still lacking, but there is evidence to suggest that CBD might even work against depression and addictive disorders. The antidepressant potential, possibly due to 5HT1A agonism, has been explored in clinical studies with impressive results in relieving anxiety and boosting mood.

Preliminary Evidence Of CBD

Our results provide preliminary evidence that CBD may affect the altered neuroendocrine as well as the psychological responses to acute stress in daily life in CHR (clinical high risk)  patients.

Therefore, in contrast to atypical antipsychotics, which have been shown to reduce elevated diurnal cortisol levels, but not correct the blunted cortisol response observed in psychosis, CBD may potentially attenuate abnormalities in some of the main components of the acute stress response in the CHR state, future research using larger samples would be required to confirm this suggestion.

These results are in line with previous research, which suggests that CBD may have anxiolytic and antipsychotic effects.
